Delta Access Program

 

The Delta Access Program is funded by the Community Access Programs of the Health Resource and Service Administration and supported by Ascension Health.

The goal of the project is to develop an integrated, coordinated, comprehensive system of care that is culturally sensitive and appropriate for the uninsured and underinsured people of Lincoln and Desha Counties of the Arkansas Delta.

The Delta Access Program is a client-centered prevention-based program to assist uninsured and underinsured to access health care. Health Care Coordinators conduct comprehensive assessments of the eligible client’s and the family’s needs for physical health, mental health and social services.

Health Care Coordinators work with clients to educate, empower, and involve them in the management of their health. Chronic illness management is emphasized with diabetes, hypertension, heart disease and asthma receiving priority.
