To request information on a CHAMPS or MASH program

1. Please go to Health Careers Home page

2. Select a program by single clicking on either CHAMPS or MASH

3. Select the city closest to you from the drop down list below the state map

4. Click Go

5. Review information on the site selected

6. Click the Request Information from link

7. Fill in the Health Careers Recruitment Information Request Form

8. Click the Submit Email to Site  button
