FINANCIAL POLICY

You as the patient, authorize treatment and are solely responsible for the payment of fees. You will be asked to settle your account at the time services are provided.  We will accept checks, cash or money orders.

•      MEDICARE and MEDICAID are accepted by this office.  State regulations require us to see your current MEDICAID card at each visit.  If we are unable to verify your eligibility for Medicaid by seeing your card, you will be responsible for payment of the account.  Medicare patients will be billed for the deductible and co-insurance.

•     This office participates in several managed care plans and files claims on your behalf.  Your identification card will be verified and co-pay (or deductible) collected at the time of service.

•      Individual insurance coverage is an agreement negotiated between you and your insurance carrier.  We will not accept responsibility for collecting or negotiating a settlement on a disputed claim. 

When you are referred to a physician outside this office for consultation services, you will be billed directly by that physician’s office.  Certain lab and hospital services are billed separately, also.  Questions about their bill should be directed to them.
