The AHEC-NE Family Practice Clinic is proud to
be associated with the Memphis VA Medical Center. This relationship brings
primary care to the veterans of Northeast Arkansas that was previously
unavailable. This portion of our Webpage is dedicated to our veteran
patients. You will find general information, helpful instructions and
directions to our clinic and the Memphis VA Hospital. While you are visiting
our site click on the links tab. There you will find information on various
medical concerns that may be of interest to you. Enjoy your stay and visit
often.

HOW TO ORDER MEDICINE REFILLS
You must use a
push button phone. Please have your social Security number and the prescription
numbers ready to enter on the keypad. This number is located on your refill
request form and medicine bottle.
If you do not
have a push button phone, you may mail in the medicine slips that you were given
when you received your medicines.

Let the phone answer
then press 2.
Enter your social
security number followed by the # sign.

SERVICE-CONNECTED VA
You are
service-connected if you received an injury or were diagnosed with a disease or
condition during your military service.
If you
become service-connected it could make you eligible for certain services for
free or at a reduced cost.
You may
apply to become service-connected for that injury, disease, or condition by
calling 1-800-827-1000. You must have a Compensation and Pension Exam.
This exam will be performed at the VA in Memphis.
CONSULTS/REFERRALS VA
Patients
who request consults/referrals to physicians outside of the VA system are
responsible for any charges incurred and will be billed directly by the
physician’s office. We recommend all referrals stay within the VA system.
PATIENT RESPONSIBILITIES VA
Follow the
Medical
Center
rules and be considerate and respectful of Medical Center Staff.
Give us
all of the information about your health as best as you can. This includes pain
and emotional problems.
Tell us
when you do not agree with the plan that the Provider gives you to follow. There
may be another way to take care of your problem.
Ask
questions when you do not know what is going on.
Keep your
appointments or notify us as soon as you know you must cancel.
Please be
patient if you have to wait. Sometimes we have emergencies
that
make us get behind. We appreciate your patience.

To enroll with the VA, please call
1-800-636-8262 ext. 7841.
TRANSPORTATION
Transportation to the Memphis VA is available
on Thursday of each week. The VA van meets at the Food Court Entrance of the
Indian Mall and leaves at 6:30 a.m. Availability is first come-first serve.
EMERGENCIES
In the event of an emergency, the patient
should determine if they can make it to the VA hospital, if not the patient
should seek emergency care by calling 911 or go to the nearest Emergency
Department. Patient is responsible for any local hospital bills they may accrue
during this time. This also includes the physician visits while in the hospital
and/or emergency room visit.
